How Admissions Work at Ezercare

At Ezercare, we’ve designed a straightforward admissions process so that individuals and families can access the care they need without unnecessary complexity. Whether you’re seeking non-medical respite housing or companion care services, our intake coordinators will walk you through eligibility, required documentation, and next steps.

Referrals are accepted from hospitals, social workers, community organizations, family members, and self-referrals. Our goal is to make the transition into care as smooth and stress-free as possible.

Step 1

Contact Ezercare

Complete Intake Form

Complete the intake application form. Gather supporting documents including ID, medical clearance, TB screening results, and emergency contact information.

Care Evaluation

Our team reviews your application and conducts a brief assessment to understand your specific care needs, preferences, and goals. We'll notify you of your eligibility within 2–3 business days.

Step 4

Admission

Once approved, we'll schedule your move-in date, finalize your personalized care plan, and ensure everything is ready for a smooth, welcoming transition into our program.
